Charting the LifeCourse
Charting the LifeCourse
This tool helps individuals with disabilities explore areas of life where they can make choices and decisions. Charting the LifeCourse also helps identify when and how much support might be needed for making and communicating decisions. Charting the LifeCourse covers all six life domains with questions in each area to prompt thought and discussion.
Person-centered planning can help your loved one make decisions and achieve a happy, fulfilled life by:
- Identifying what they do and do not want
- Identifying challenges that need to be overcome
- Discussing strengths and limitations
- Setting goals that move them toward the life they want
- Letting go of habits and actions that do not lead to a happy, fulfilled life
The UPC’s Person-Centered Planning website page will also walk you through the steps for Person-Centered Planning and how to create a vision for a good life. The UPC’s Self-Determination website page details the components of self-determination and why it is important for each individual to have the opportunity to create the life they envision for themselves.
A summary of Charting the LifeCourse, Self-Determination, Person-Centered Planning, and Community Mapping can be found in the GREEN section of the Transition University website and the GREEN section of the Youth Transition Book, and the Choices Book.
