The System of Care (SOC) is a statewide program under the Utah Department of Health and Human Services (DHHS) designed to support youth and families facing serious mental health challenges.
With the help of peer support specialists and the High Fidelity Wraparound process, youth and families receive individualized support that helps them remain in their homes, schools, and communities.

What is The System of Care?
The System of Care is a family-driven, youth-guided approach to mental health support. Instead of families navigating systems alone, SOC brings together professionals, peer support specialists, and trusted natural supports to create a coordinated plan that reflects what matters most to the family.

Coordinated Mental Health Support
System of Care is a program that helps youth and families coordinate mental health services across providers and systems.

Family-Centered Planning
Families work with a team to create an individualized plan based on their specific needs, strengths, and goals.

Peer and Professional Support
Families receive support from trained peer specialists and professionals who help guide decision-making and service access.

Community-Based Care
Supports are designed to help youth remain in their homes, schools, and communities whenever possible.
