Maternal and Child Health’s Family Needs Assessment: Help Determine Priorities for Children with Special Health Care Needs

 

The Utah Department of Health (UDOH) Bureau of Children with Special Health Care Needs is conducting a needs assessment. The aim of the needs assessment is to help UDOH understand the experiences and needs of children with special health care needs and their families. The information you provide will help UDOH prioritize and plan services for children with special health care needs in the state. This survey is completely voluntary, anonymous, and should take 10 – 15 minutes to complete.

 

Children and youth (ages Birth – 22) with special health care needs are defined as: “those who have or are at increased risk for a chronic physical, developmental, behavioral, or emotional condition and who also require health and related services of a type or amount beyond that required by children generally”.
